IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Form 1095-B for the 2025 Tax Year is Available Upon Request
Due to new federal legislation designed to reduce paperwork and administrative burden, we are no longer required to automatically mail a paper copy of Form 1095-B to all covered individuals. This form provides information about the health coverage you were enrolled in and serves as proof of your minimum essential coverage. You are not required to attach this form to your personal tax return, but you should keep it with your tax records if you receive one. If you would like to request a free paper copy of your Form 1095-B for your records, you may do so using one of the following methods:
A paper copy of your form will be mailed to the address on file within 30 days of receiving your request. This notice will remain available on our website through October 15, 2026.
